Identity theft can affect your credit, create fraudulent debt or false medical records, and much more.Identity theft is a crime that can happen to anyone and involves your personal information being stolen through methods as simple as stealing your documents or as sophisticated as “phishing” or spyware.
Requirements:
- Please complete an Identity Theft Packet Form and print it.
- This form provides us the information we need to make an offense. It must be filled out with as much information as possible. - Please have the form signed and notarized or witnesses.
- Please fax, mail, or scan and email the completed form.
